Please have all the following information available when you begin as you are not able to save the registration form while in process 为了确保您能够顺利填写注册表格，建议您在填写之前准备好以下所有信息:

• Contact information for the primary and secondary caregiver 主要及次要监护人的联系信息;

• REQUIRED 必填: At least 2 additional Emergency Contacts (must be 18+) full names, addresses,
  phone numbers, email address of that person or you can list your email address again 额外提供至少2名紧急联系人(年满18岁) 信息，包括其全名、地址、电话号码、电子邮箱 (可再次填写您自己的电子邮箱);

• OPTIONAL 选填: Contact information for any additional adult (18+) that is authorized to pick up your
  child from the program 授权接送您孩子的其他成年人 (年满18岁) 的联系信息;

• Child's height and weight in inches 孩子的身高 (英寸) 和体重 ;

• Child's medical information: allergies, disabilities, medical conditions, special needs, use of an epi-pen, inhaler or other medications 孩子的医疗信息：过敏、残疾、疾病情况，特殊需求，是否使用肾上腺素注射器、吸入器或其他药物;

• Child's doctor's name and telephone number 孩子的医生姓名及电话号码;

• Health Insurance company name and policy number 健康保险公司名称及保险号码;

• Credit/debit card for registration fee 支付注册费的信用卡/借记卡;

• OPTIONAL but recommended 选填但建议填写: credit/debit card or bank draft/checking account information to set up weekly auto payments 信用卡/借记卡或银行汇票/支票账户信息，以设置每周自动付款。

A non-refundable $25 registration fee per child is due when submitting your registration, payable by credit card or debit card. Please note a 3 day minimum is required in order to process your enrollment. 为每位孩子提交注册时，需缴纳25美元注册费，不可退还，可通过信用卡或借记卡支付。请注意，处理您的注册需要至少3天的时间。
